Match Report Procedure

CPSL Match Report Procedure NEW AND REVISED 2/17/2008

1. Prior to arrival at the match site, the referee is responsible for downloading and printing a blank USSF Match Report Form. This form includes the Supplemental Form.
   Note: This form is available at www.ussoccer.com at Referees, www.thearbiter.net at Forms, www.johnson-resources.com/soccer.htm. If you do not have access to a printer, contact me and I have plenty of forms for you to use and copy.
2. Prior to the kickoff, inspect each team, match Player Passes to the team's printed roster, and retain Player Passes and roster.
3. At the end of the match, retain the team rosters, retain the Player Passes of any player SENT OFF, return all the other Player Passes, and fill out the Match Report. Each crew member must sign the Match Report.
   Go online to www.johnson-resources.com/soccer.htm and follow the link to CPSL Match Report Submission. After you have filled out the online Match Report, press SUBMIT. Go online to www.thearbiter.net and click on the Match Report icon for your game. From your written match report fill out the online Arbiter Match Report and Save. Be precise and accurate, remembering League officials will be reading these reports.
   Here is the format for reporting misconduct:
   YC/RC; HT/VT; #; Lastname, Firstname; CODE; MM'; Reason
   For example:
   YC; VT; #13; Smith, Sean; FRD; 83'; Prevented quick kick.
   Use this format for Cautions and Send Offs, and you are still required to follow the Supplemental Match Report procedure below for Send Offs.
4. If you had a SEND OFF, please download and print out the Supplemental Match Report, also available at the above sites. Make a copy of both your original Match Report and the Supplemental Report. Mail in: Match Report, Supplemental Report and Player Pass of the offending player to:

It is advisable that you keep completed Match Reports and rosters at least until the end of the current season.
